Job SummaryWe are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Community Care Specialist. As a key member of our care team, you will provide high-quality care and support to our residents, working closely with our Certified Nursing Assistants and other healthcare professionals.
Responsibilities- Provide hands-on care to residents, including medication administration and performance of treatments
- Develop close relationships with residents and their families, providing exceptional customer service and utilizing courtesy, dignity, and respect
- Lead by example for our unlicensed caregivers and volunteers, promoting a culture of teamwork and compassion
- Collaborate with our care team to design and implement individualized care plans, ensuring the best possible outcomes for our residents
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments
- Graduate of an accredited practical nursing program
- Licensed as a Licensed Practical Nurse (LPN) in good standing by the state in which the community is located
- Experience with general nursing and elder care
- Working knowledge of the Home Care Bill of Rights as it applies to the LPN
- Knowledge of medication administration and performance of treatments
- Possess compassion for and commitment to hospitality, service, and excellence in elderly care
- Ability to teach policies/procedures under appropriate direction
- Ability to function as a primary caregiver and perform duties of unlicensed team members
- Ability to maintain information as confidential and private
- Ability to remain calm, kind, and patient during difficult circumstances and emergencies
- Ability to set appropriate boundaries in working with residents and their families while simultaneously being approachable and empathetic
