This person works to maximize the patient's health outcomes and experience by identifying patient and family needs, completes pre-visit planning, supports clinic visits, and provides closure to the clinic visits.The Care Coordinator utilizes coaching to help ensure patient and family participation in treatment plans and provides exceptional customer service to internal and externalcustomers (providers, clinic staff, patients, and families).

OurFairview Southdale Cardiology Clinicsupports a wide variety of patients with their cardiac needs. This includes preventative care, coronary artery disease, hypertension, heart failure, atrial fibrillation and more. We have a large group of providers that work closely with the RN's, clinical support staff, scheduling staff and more to provide excellent patient care. Southdale was rated #1 in the twin cities for cardiac care, and coronary intervention. We are excited to add an individual to our clinic who enjoys being part of a team and has a passion for caring for patients

What you will do as an RN Care Coordinator:

  • Coordinates care across the health care continuum within designated sub-specialty population(s)
  • Serves as single point of contact by facilitating and overseeing transitions in care within and between care settings, providers, and services
  • Completes pre-visit planning and post visit wrap-up and determines necessary follow-up appointments and/or treatments to optimize patient visit
  • Facilitates proactive disease management and preventative health care using evidenced based practice
  • Performs other duties as determined by manager and clinic needs

What you will need:

  • Current Minnesota RN license
  • BLS certification
  • 3+ years of clinical nursing experience
  • Demonstrated experience in patient advocacy and education, coaching, and proactive disease management
  • EMR and keyboarding/computer skills

Hours:0.8 FTE, 32 hours per week
