Registered Nurse Case Manager - Northeast Region

As a Registered Nurse (RN) Case Manager, your role will involve working closely with interdisciplinary teams throughout the healthcare continuum to ensure effective care transitions.

Key responsibilities include advocating for patients, conducting comprehensive assessments, and developing care plans that empower patients and their families.

Case Managers are also responsible for adhering to the standards set by accrediting organizations, state regulations, and CMS Conditions of Participation.

Key Responsibilities:
  • Facilitate inter-professional collaboration during patient care episodes and ensure smooth transitions to the next level of care.
  • Proactively identify and address barriers to effective patient transitions.
  • Collaborate with patients and their support systems to create personalized transitional care plans, participating in relevant meetings and activities.

Education Requirements:
  • Graduate from an accredited nursing program.
  • Bachelor of Science in Nursing (BSN) is preferred.

Certifications:
  • Current State Licensed Registered Nurse (RN).
  • Certification from an accredited body is preferred within one year of eligibility (e.g., CCTM, ACM, CCM).
  • Basic Life Support (BLS) certification through the American Heart Association (AHA).

Competencies:
  • Strong critical thinking and decision-making skills.
  • Ability to work effectively under pressure.

Experience:
  • Minimum of three years of recent clinical nursing experience required.
  • At least two years of experience in Utilization Review, Quality, or Care Management is preferred.

Physical Requirements:
  • Ability to communicate effectively and perform tasks in a stationary position for extended periods.
  • Ability to navigate the hospital system and manage physical records.
  • May require lifting up to 10 pounds in certain locations.

Personal Protective Equipment:
  • Adhere to Standard Precautions and utilize personal protective equipment as necessary.
