- Key Responsibilities:
- Facilitates inter-professional collaboration throughout the acute care process, ensuring effective transitions to the next level of care or community services.
- Proactively identifies and addresses barriers to patients' transitions of care.
- Partners with patients and their care partners to create a tailored, comprehensive transitional care plan. Participation in patient rounds, huddles, and relevant committee meetings is expected.
- Education:
- Graduate from an accredited nursing program.
- BSN is preferred.
- Certifications:
- Current State Licensed Registered Nurse (RN).
- Certification from an approved organization is preferred within one year of eligibility, such as CCTM, ACM, or CCM. Basic Life Support (BLS) certification through the American Heart Association is required.
- Competencies:
- Requires critical thinking, decisive judgment, and the ability to work independently.
- Must be capable of functioning in a high-pressure environment and taking appropriate actions.
- Work Experience:
- A minimum of three years of recent clinical RN experience is required. At least two years of experience in Utilization Review, Quality, or Care Management is preferred.
- Familiarity with acute care regulatory and accreditation standards is preferred, along with experience in discharge planning or Quality Improvement.
- Physical Requirements:
- Normal or corrected hearing and vision within the normal range.
- Ability to remain in a stationary position for extended periods.
- Capable of communicating and exchanging accurate information.
- Proficient in handling physical records and operating office equipment.
- Ability to travel within the healthcare system.
- In certain locations, the ability to lift up to 10 pounds may be required.
- Personal Protective Equipment:
- Adheres to Standard Precautions by utilizing personal protective equipment as necessary for procedures.
