About the Role

In this role as a Hospice and Palliative Care Coordinator, you will work with patients and their families to identify and explore options for managing chronic illnesses. You will develop and implement personalized care plans that address the unique needs of each patient.

You will work closely with interdisciplinary teams to provide medical crisis counseling, case management, and palliative care services. This role requires a strong understanding of hospice and palliative care principles, as well as excellent communication and interpersonal skills.

The ideal candidate will have a Master's degree in Social Work and be licensed as a Licensed Independent Social Worker (LISW) in the State of New Mexico. Five years of experience in hospital social work or discharge planning is also required.
