Your Role
As a Hospice RN/RN Case Manager, you will play a vital role in providing direct patient care services to hospice patients and supportive care to the patient's family. You will also act as primary liaison between physician, patient/family and hospice team. Your responsibilities will include reporting directly to the Administrator, Executive Director or Patient Care Manager Senior and maintaining primary control and professional management of each patient.

Requirements
To be successful in this role, you will need to demonstrate flexibility and composure while supporting others in a time of crisis. You will also need to have a RN license in the state of requested employment (or eligibility to obtain) and at least 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology.
