Palliative Care Specialist

We are seeking an experienced Palliative Care Specialist to join our team. As a Palliative Care Specialist, you will be responsible for providing direct patient care and case management of Hospice patients in outpatient or inpatient settings.

About the Role

The ideal candidate will have a strong background in nursing and experience working with Hospice patients. You will be responsible for developing, coordinating, and scheduling delivery of care through a multidisciplinary team to the patient in the inpatient, home, or community setting.

Responsibilities

  • Develop, coordinate, and schedule delivery of care through a multidisciplinary team
  • Ensure that end-of-life care is delivered within quality and utilization guidelines
  • Coordinate with the multidisciplinary team, support services, and community resources to ensure plan of care goals are achieved

Qualifications

  • Associate Degree in Nursing
  • State of New Mexico or Compact State Nursing License
  • BLS certification REQUIRED at time of hire
  • Minimum one year in direct patient care setting or home health care
