Position Overview
The Heart Failure Care Coordinator plays a crucial role in managing patient care throughout the healthcare continuum, ensuring adherence to the provider's care plan and national standards within the nursing scope of practice. This position is vital in educating patients about heart failure and their treatment options, facilitating a comprehensive understanding of their health journey.
Key Responsibilities
- Coordinate patient care effectively, aligning with the prescribed treatment plans and nursing practices.
- Provide education and support to patients, enhancing their understanding of heart failure and associated treatment protocols.
- Monitor patient outcomes and engage in quality improvement initiatives to elevate patient care standards.
- Collaborate with healthcare team members to positively influence patient outcomes and experiences.
- Ensure seamless communication between referring physicians and care providers to expedite treatment planning.
- Oversee the Meds to Beds program and assist in initiating prior authorizations for discharge medications.
- Conduct 30-day post-discharge tele-monitoring for heart failure patients.
- Act as a bridge between inpatient and outpatient settings, coordinating follow-up appointments within a week of discharge.
- Bachelor of Science in Nursing with three (3) years of clinical experience in Cardiology, or an Associate Degree in Nursing with five (5) years of clinical experience in Cardiology.
- Valid Registered Nurse license in the state of Georgia.
- Must obtain Heart Failure Certification (CHFN) within twelve (12) months of employment.
- Strong understanding of service lines and patient support programs.
- Heart Failure Certification (CHFN) through the American Association of Heart Failure Nurses Certification Board.
