Care Coordination Specialist

**GeorgiaGov** has partnered with the Georgia Department of Behavioral and Developmental Disabilities (DBHDD) to focus on assisting youth in need, ages 6-21 years of age, who are experiencing health crises and barriers to accessing appropriate care and support, throughout a continuum of care.
This partnership allows us to implement a formalized multi-agency care coordination protocol focused on removing barriers and expediting access to the right care at the right time for youth and families.
Mobilizing Navigator - Care Coordinator
**Key Responsibilities:**
- Develop and implement care coordination protocols to remove barriers and expedite access to care
- Collaborate with multi-agency teams to provide comprehensive care and support to youth and families
- Conduct assessments and develop individualized care plans to address health crises and barriers to care
- Provide education and support to youth and families on accessing and navigating the healthcare system
- Monitor and evaluate the effectiveness of care coordination protocols and make recommendations for improvement
Requirements:
- High School diploma/GED
- Three (3) years of job-related experience in a social services-related position
- Or One (1) year of experience at the lower level Social Svcs Tech 2 (SST011) or equivalent position
