Care Coordinator

View Point Health is seeking a highly motivated and dedicated Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in enhancing client and family engagement in services, providing initial screenings, and connecting clients to needed resources.
Key Responsibilities:- Provides direct care coordination to enhance client and family engagement in services
- Conducts initial screenings of client needs and assists clients in connecting to needed resources
- Answers and screens telephone and email inquiries in a professional and courteous manner to provide clear and accurate program information
- Responsible for engagement activities to facilitate active connection to services and improve behavioral health outcomes
- Uses social determinants of health and other screening/monitoring tools to identify client and family needs
- Collects program-specific information using the electronic health record to document activities and advocacy efforts
- Establishes and maintains resource connections with external partners and community organizations, manages referral procedures, and initiates and tracks linkage to care
- Provides support with arranging primary care and other health appointments, therapy, and increases access to other resources that promote integrated care and family wellness
- Collaborates with interdisciplinary teams to reduce behavioral health disparities and increase overall engagement in care
- Bachelor's degree in a Social Services-related field from an accredited college or university
- One year of experience in a Social Services-related field
- Ability to lift 20 pounds, requires long periods of sitting/standing
- Must be 21 years of age
- Must have a valid Georgia driver's license and Motor Vehicle Record in accordance with company policy
