Clinical Care Specialist

Acadian Health is seeking a highly skilled Clinical Care Specialist to join our team. As a Clinical Care Specialist, you will be responsible for providing high-quality patient care and management in a variety of settings.
Key Responsibilities:- Conduct thorough environmental and clinical assessments to identify patient needs and develop effective care plans.
- Provide non-emergency clinical care in non-traditional settings, with a focus on chronic disease management.
- Work collaboratively with virtual care teams to coordinate patient care and ensure continuity of care objectives.
- Respond to crisis calls and provide clinical care as directed by protocol and medical control.
- Conduct regular home visits to provide ongoing interaction with patients and their caregivers.
- Develop and implement disease-specific education programs for patients and their caregivers.
- Communicate effectively with multiple agencies and healthcare providers to ensure seamless care coordination.
- Maintain accurate and timely patient medical records using electronic health record software.
- Make sound decisions in critical situations and prioritize patient care needs.
- Current BLS and ACLS certifications; PHTLS and PALS certifications preferred.
- Required licensure to practice as a Registered Nurse in the state of employment.
- Ability to obtain Community Paramedicine (CP-C) certification or licensure within one year of hire date.
- Effective verbal, written, and interpersonal communication skills.
- Demonstrated ability to provide effective coaching and leadership.
- Demonstrated teaching and educational facilitation skills.
- Ability to adhere to established standards for educational quality.
- Familiarity with basic computer applications and functions.
- Team-oriented and able to work effectively with others.
- Degree from an accredited college or university.
- Previous trainer or instructor experience.
- Previous Critical Care experience.
- Maintain social, ethical, and organizational norms.
- Ability to accomplish all task details, no matter how small.
- Maintain good personal motivation and develop a sense of ownership of job tasks and results.
- Ability to work under pressure and meet strict deadlines.
- Excellent interpersonal and communication skills.
- Orientation to job requirements.
- Completion of the Acadian Health clinical education program.
- Completion of ride-along shifts with current Acadian Health staff.
- Clearance to perform issued by an Acadian Health Operations Supervisor.
- Obtain Community Paramedicine certification or licensure in the state of employment.
- Participation in the Acadian Health continuing education program.
