Certified Home Health Aide for Home Visits

We are seeking a compassionate and dedicated Certified Home Health Aide to join our Home Healthcare Team at Physicians Choice Home Health. As a Certified Home Health Aide, you will play a vital role in providing quality home health visits to our patients in the comfort of their own homes.
Responsibilities:- Provide hands-on personal care, including bathing, back rubs, oral hygiene, shampoos, and changing bed linen.
- Assist patients with dressing and undressing, skin care to prevent breakdown, and toileting activities.
- Keep patient living areas clean and orderly, plan and prepare nutritious meals, and assist with feeding patients as needed.
- Take and record oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure when ordered.
- Assist patients in ambulation and exercise according to the plan of care, and perform range of motion and other simple procedures as an extension of therapy services.
- Meet safety needs of patients and use equipment safely and properly.
- Report on patient condition and significant changes to the assigned nurse.
- Meets the training requirements and successfully completed a training and competency evaluation program.
- Shows proof of registration on the Home Care Aide Registry through the Department of Social Services, Home Care Services Bureau, preferred.
- At least 18 years of age.
- Ability to read and follow written instructions and document care given.
- Self-directing with the ability to work with little direct supervision.
- Empathy for the needs of the ill, injured, frail, and impaired.
- Possess and maintain current CPR certification.
- Discretion in dealing with sensitive and confidential information.
- Ability to travel to different patients' homes.
