Care Coordinator

We are seeking a highly skilled and compassionate Care Coordinator to join our team at People Care. In this role, you will be responsible for providing exceptional customer service to patients and their families.
About the Role
- Answer incoming calls and provide assistance to clients in a timely and professional manner.
- Gather necessary information from clients using phone systems to ensure accurate documentation.
- Conduct initial assessments to determine client needs and provide solutions.
- Document client information accurately and efficiently to support effective care coordination.
- Collaborate with other team members to ensure smooth intake process and excellent customer service.
Requirements
- Previous experience in a similar role is preferred but not required.
- Strong communication skills, both verbal and written, to effectively interact with clients and colleagues.
- Excellent organizational and multitasking abilities to manage multiple tasks and prioritize client needs.
Benefits
We offer a competitive salary of $45,000 - $55,000 per year, depending on experience, as well as opportunities for professional growth and development. Our comprehensive benefits package includes health insurance, paid time off, and a 401(k) plan.
