About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Maternal Health Case Manager. As a key member of our team, you will play a vital role in promoting healthy pregnancy, delivery, and postpartum outcomes for mothers and babies.

Key Responsibilities
  • Provide evidence-based education to pregnant women and new mothers on healthy pregnancy, delivery, and postpartum care.
  • Develop and implement individualized care plans to address the unique needs of each patient.
  • Collaborate with healthcare providers, community organizations, and other stakeholders to ensure seamless care coordination.
  • Conduct home visits and provide support to patients in their homes, as needed.
  • Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
Requirements
  • Current, unrestricted licensure as a Registered Nurse in Kansas.
  • 2+ years of experience working in maternal and child health.
  • 1+ years of experience with MS Office, including Word, Excel, and Outlook.
  • Ability to travel up to 75% of the time to visit patients in their homes and other settings.
  • Access to a designated quiet workspace in your home with the ability to secure Protected Health Information (PHI).
Preferred Qualifications
  • Knowledge of the culture and values of community-based care.
  • Case management experience.
  • Background in managed care.
  • Bilingual (Spanish).
