Caregiver Coordinator

Responsibilities and Expectations:
The successful candidate will be responsible for implementing plans of care, administering medications, and maintaining accurate records. You will work closely with our multidisciplinary team to ensure that each consumer receives personalized care and attention. Your expertise in medication administration, documentation, and communication will be invaluable in maintaining accurate records and reporting incidents as needed.
Key Performance Indicators:
* Implement plans of care for consumers, including medication administration and daily living assistance
* Maintain accurate records, including MARs, progress notes, and incident reports
* Provide daily living assistance, including bathing, dressing, feeding, and toileting
* Collaborate with the multidisciplinary team to ensure high-quality care and support
* Work at the group home during scheduled hours, even if the consumer is not present
Working Conditions:
* The successful candidate will work in a fast-paced residential setting, interacting with consumers, caregivers, and other professionals
* Must be able to lift 25 pounds and perform physical tasks as needed
* Regular attendance and punctuality are essential for this role
