About the Role:

We are seeking an experienced Transplant Coordinator to join our team at Duke University Hospital. As a key member of our interdisciplinary team, you will play a crucial role in delivering exceptional patient care and advancing health together.

Job Summary:

The Transplant Coordinator will be responsible for coordinating the care of adult liver and kidney transplant patients, from pre-transplant evaluation to post-transplant follow-up. This includes educating patients and families on all aspects of post-transplant care, arranging follow-up appointments and labs, and working with the multidisciplinary team to address patient-specific needs. The ideal candidate will have 5 years of nursing experience, with at least 2 years in transplant, critical care, or chronic disease-specific settings. A BSN is required, and an MSN is preferred.

Responsibilities:

* Coordinate daily transplant rounds with the transplant team, discussing clinical, psychosocial, or educational concerns regarding fresh pre and post-transplant patients
* Educate patients and families on all aspects of post-transplant care, including arranging follow-up appointments, labs, and working with the multidisciplinary team on patient-specific needs
* Communicate with patients and caregivers regarding the recovery process and facilitate discharge planning needs
* Compose written transplant education summaries to send to the post-transplant coordinator, provide patient or caregiver work letters, documenting education done in the Electronic Medical Record, the outcome of education
* Conduct post-discharge transplant calls 24-48 hours after discharge to review education, discuss future appointments, and address any outstanding concerns or questions
* Document inpatient summaries to send to the post-transplant coordinator about care provided in hospital and changes made to the plan of care
* Ability to make clinical judgments based on synthesis of current research, concepts, principles, theories, and outcomes to solve complex problems
* Collaborate with medical faculty, pharmacy, nursing personnel, case management, and other health disciplines to facilitate meeting patient and family needs
* Staying current on post-transplant research and applying evidence-based research and recommendations to patients/families on care of the transplant patient
* Complete regulatory documentation in UNOS database/hospital EMR
* Reviewing education materials and working with department if there are changes in current practice

Requirements:

* Current or compact RN licensure in the state of North Carolina required
* BLS required
* 5 years nursing experience with at least 2 years in transplant, critical care, or chronic disease-specific settings
* BSN required, MSN preferred
