About the Role

The Transplant RN Coordinator plays a vital part in delivering compassionate care to patients undergoing organ transplantation at Duke Health. With a strong focus on patient-centered care, this role involves coordinating with multidisciplinary teams to ensure seamless transitions and effective post-transplant recovery.

Key Responsibilities:

  • Educating patients and families about post-transplant care, including medication management, follow-up appointments, and rehabilitation programs
  • Communicating with caregivers and healthcare providers to facilitate discharge planning and address any concerns or questions
  • Documenting patient progress and summarizing transplant education in the Electronic Medical Record (EMR)
  • Conducting post-discharge calls to review education and provide support as needed
  • Collaborating with medical faculty, pharmacy, and other healthcare professionals to ensure optimal patient care

Requirements

  • Bachelor of Science in Nursing (BSN) degree required; Master of Science in Nursing (MSN) preferred
  • At least 5 years of nursing experience, with 2+ years in transplant, critical care, or chronic disease-specific settings
  • Current RN licensure in North Carolina and Basic Life Support (BLS) certification

Compensation

Estimated annual salary: $85,000 - $110,000 based on experience and qualifications.
