Position Title: F-PATH Residential Case Manager - Overnight Shift
FLSA Status: Non-Exempt
Compensation: $28.84 per hour
At Community House Mental Health Agency, we are dedicated to fostering a nurturing environment for individuals facing severe and persistent mental health challenges. Our mission is to ensure that every individual who seeks our assistance feels acknowledged and valued.
The F-PATH program, part of our Diversion Services, focuses on assisting adults with significant behavioral health issues. This voluntary outreach initiative is designed for individuals who have encountered multiple requests for competency evaluations recently. Our dedicated F-PATH team offers complimentary, companion-based case management aimed at connecting clients to essential resources and long-term behavioral health treatment, ultimately helping to reduce cycles of incarceration.
Key Responsibilities:
- Establish rapport with clients facing significant barriers to accessing services.
- Support clients in developing essential life skills, including meal preparation, laundry, and personal hygiene.
- Facilitate transportation for clients to necessary appointments.
- Assist participants in engaging with long-term services that promote self-sufficiency and recovery.
- Provide immediate crisis intervention and resolution as required.
- Conduct risk assessments and aid clients in developing de-escalation strategies.
- Collaborate with other professionals in behavioral health, legal, and medical fields.
- Support the intake and interview process for F-PATH Emergency Housing.
- Assist clients in fulfilling legal obligations to help reduce recidivism.
- Engage with clients who may be under the influence of substances or experiencing severe poverty.
- Utilize a harm reduction approach, demonstrating a commitment to serving individuals with co-occurring mental health and substance use disorders.
- Advocate for clients and empower them to advocate for themselves.
- Ensure timely completion of documentation in accordance with program standards.
- Adhere to HIPAA and confidentiality regulations.
- Perform additional duties as assigned.
Qualifications:
- Bachelor's degree in a related social services field.
- Legible handwriting is essential.
- Valid driver's license with a clean driving record for agency vehicle use.
- Experience with individuals facing severe mental health challenges, chronic substance use issues, and involvement in the criminal justice system is preferred.
- Strong diagnostic and assessment skills, particularly with co-occurring disorders.
- Familiarity with the forensic and competency systems is advantageous.
- Ability to work a flexible schedule, including some holidays.
- Basic understanding of the criminal justice system.
- Cultural sensitivity and awareness of diverse populations.
- Eligibility to obtain a Washington State Agency Affiliated Counselor License.
- Capability to perform the duties outlined in this description with reasonable accommodations.
