Job Title: Residential CaregiverJob Summary

Community House Mental Health Agency is seeking a compassionate and dedicated Residential Caregiver to join our team. As a Residential Caregiver, you will provide comprehensive residential treatment and coordination of other medical and rehabilitative treatment for adults living in community-based supported housing programs.

Responsibilities
  • Provide assistance in Activities of Daily Living to residents as per their service plan
  • Coordinate resident needs, including assistance with medical appointments, outside agency services, and therapy as needed
  • Accompany clients to appointments as necessary
  • Visually supervise medication distribution as required
  • Maintain member records in accordance with agency, county, state, and federal guidelines
  • Provide input to residents' primary care providers and/or case managers on resident progress and treatment plans
  • Facilitate group activity sessions and specialized resident events
  • Provide immediate crisis intervention and resolution as needed
  • Provide 1:1 resident contact on a weekly basis, as per individual service plans
  • Assist in admission and discharge planning as needed
  • Review daily logs and documents after each shift and communicate with other staff members regarding emergency situations
  • Maintain and update administrator checklists of client treatment plans and review dates for administrator review
  • Attend scheduled staff meetings, community meetings, and when on duty or assigned
  • Monitor facility milieu for visitors, TV time, and quiet times
  • Prepare resident meals when the cook is not on duty and per menu
  • Lead assigned life skills groups
  • Coordinate fire drills, resident orientation, chores, and resident room maintenance during resident move-in and move-out
  • Record progress notes in clients' charts within 7 days maximum of each individual/group activity with residents
  • Commit to 12 hours of annual continuing education/training
  • Maintain resident and staff confidentiality at all times
Requirements
  • High School Diploma/GED
  • Current First Aid and CPR certification
  • Current Food Handlers Permit
  • Legible handwriting
  • Ability to perform simple mathematics
  • Ability to read and comprehend agency/member charts
  • Ability to work non-traditional schedules as required for supporting clients at their housing site, including holidays as assigned
  • Valid driver's license and clean driving record to use agency vehicles
  • Experience with hands-on caregiving and/or ability to acquire specialized training to work with the target population
  • Fundamentals/Modified Fundamentals of Caregiving and ability to obtain other specialized training per WAC
  • Able to obtain a certificate from DSHS 75-hour training within 120 days of employment and other specialized trainings per WAC
  • Ability to perform duties outlined in the description within reasonable accommodation
