Putnam Farms is in search of a dedicated LPN to enhance our nursing team. As a Licensed Practical Nurse, your primary responsibility will be to provide exceptional nursing care to our residents in a nurturing and supportive environment.

LPN Responsibilities:
  • Conduct thorough assessments, communicate effectively, and document residents' health status.
  • Monitor and report any significant changes in resident behavior and health to the appropriate personnel.
  • Oversee and guide the daily tasks of Resident Care Assistants.
  • Mentor and support Lead Resident Care Assistants in their roles.
  • Coordinate care requirements with community healthcare providers through efficient case management.
  • Balance service demands while promoting resident independence.
  • Identify and assist in preventing elder abuse, neglect, and exploitation, reporting to the necessary authorities.
  • Respond to emergencies and manage personal emergency response systems, knowing when to seek additional support.

Qualifications for LPN:

  • Current state licensure as an LPN and CPR certification
  • Graduation from an accredited LPN program
  • 1 to 2 years of nursing experience preferred
  • Experience working with the elderly in any capacity is highly valued
