Transplant Coordinator

We are seeking an experienced Transplant Coordinator to join our team at Duke University Hospital. As a key member of our interdisciplinary team, you will play a vital role in delivering exceptional patient care and ensuring a seamless transition for our patients and their families.
Responsibilities- Provide comprehensive education to patients and families regarding post-transplant care, including arranging follow-up appointments, labs, and collaborating with the multidisciplinary team to address patient-specific needs.
- Communicate effectively with patients and caregivers regarding the recovery process and facilitate discharge planning needs.
- Compose written transplant education summaries to send to the post-transplant coordinator, provide patient or caregiver work letters, and document education in the Electronic Medical Record.
- Document inpatient summaries to send to the post-transplant coordinator about care provided in the hospital and changes made to the plan of care.
- Collaborate with medical faculty, pharmacy, nursing personnel, case management, and other health disciplines to facilitate meeting patient and family needs.
- Stay current on post-transplant research and apply evidence-based research and recommendations to patients/families on care of the transplant patient.
- Complete regulatory documentation in the UNOS database/hospital EMR.
- Review education materials and work with the department if there are changes in current practice.
We are looking for a highly skilled and experienced Transplant Coordinator with at least 5 years of nursing experience, including at least 2 years in a transplant, critical care, or chronic disease-specific setting.
