About the Role

We are seeking an experienced Transplant Coordinator to join our team at Duke University Hospital. As a key member of our interdisciplinary team, you will play a vital role in delivering exceptional care to our patients and their families.

Key Responsibilities
  • Attend daily transplant rounds with the transplant team to discuss clinical, psychosocial, or educational concerns regarding fresh pre and post-transplant patients for adult liver and kidney.
  • Educate patients and families regarding all aspects of post-transplant care, including arranging follow-up appointments, labs, and working with the multidisciplinary team on patient-specific needs.
  • Communicate with patients and caregivers regarding the recovery process and facilitate discharge planning needs.
  • Compose written transplant education summaries to send to the post-transplant coordinator, provide patient or caregiver work letters, and document education done in the Electronic Medical Record.
  • Conduct post-discharge transplant calls 24-48 hours after discharge to review education, discuss future appointments, and address any outstanding concerns or questions.
  • Document inpatient summaries to send to the post-transplant coordinator about care provided in the hospital and changes made to the plan of care.
Requirements
  • Current or compact RN licensure in the state of North Carolina required.
  • 5 years of nursing experience with at least two years in transplant, critical care, or chronic disease-specific settings.
  • BSN required, MSN preferred.
