Job Summary

La Clinica de Familia Inc. is seeking a highly motivated and compassionate Certified Community Health Worker to join our team. As a key member of our population health efforts, you will play a vital role in supporting patients in accessing healthcare services, providing social support, and promoting health education.

Key Responsibilities
  • Provide telephonic and in-person support to patients, identifying barriers to accessing healthcare and connecting them with community resources.
  • Work closely with healthcare providers to ensure seamless care coordination and address social determinants of health.
  • Develop and maintain relationships with community partners to enhance access to care and improve health outcomes.
  • Conduct home visits and provide education on healthy behaviors, disease management, and wellness strategies.
  • Collaborate with multidisciplinary teams to develop and implement population health initiatives.
Requirements
  • Certified Community Health Worker (CCHW) certification required.
  • 1+ years of experience in a healthcare setting, preferably in a community-based or outreach role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Proficient in Microsoft Office Suite and electronic medical records.
