Position Overview:
As a vital member of our healthcare team, the Registered Nurse Case Manager will work under the supervision of the Clinical Nurse Manager to deliver exceptional nursing services aimed at promoting the overall health and well-being of patients. This role involves monitoring patient outcomes and collaborating with physicians and other healthcare professionals to ensure effective home health care plans.
Key Responsibilities:
- Conduct comprehensive assessments to identify the physical, emotional, socio-economic, and environmental needs of patients and their families.
- Administer direct skilled nursing care in alignment with physician directives, ensuring timely documentation of all care provided.
- Continuously observe and evaluate patients' physical and mental conditions, reporting any changes to the physician and managing care transitions as necessary.
- Provide education and counseling to patients, families, and caregivers to support health management.
- Engage in quality assurance initiatives and participate in agency-sponsored training programs to enhance service delivery.
- Collaborate with patients, their representatives, caregivers, and physicians to develop, implement, and assess individualized care plans.
- After the initial 90 days, oversee and guide the clinical activities of LPNs, ensuring compliance with established protocols.
- Identify additional home health service needs for patients and facilitate appropriate referrals under the guidance of the Clinical Nurse Manager.
- Adhere to all compliance requirements associated with the role.
- Perform other related duties as assigned.
Qualifications:
Required: Strong research capabilities and effective interpersonal communication skills are essential. Familiarity with systems integration is also necessary.
Preferred: A Bachelor's Degree in Nursing and experience in a qualified home health agency.
Working Conditions:
Manual: Requires significant manual dexterity and coordination.
Occupational: Involves some occupational risks.
Physical Effort: Requires very heavy physical effort, with the ability to exert up to 50 lbs. of force.
Physical Environment: May involve exposure to various environmental factors such as dirt, odors, noise, and human waste.
