RN Case Manager

We are seeking a skilled and compassionate Registered Nurse to join our home healthcare team at Nuvance Health. As a Case Manager RN, you will play a vital role in delivering high-quality patient care and coordinating services to meet the unique needs of our patients and their families.
Responsibilities- Assess and evaluate the physical, emotional, and socio-economic needs of patients and their families.
- Develop and implement individualized care plans in partnership with patients, families, and healthcare professionals.
- Provide education and support to patients and caregivers to promote self-management and independence.
- Coordinate services with other healthcare providers and agencies to ensure seamless care transitions.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Current RN license in the State of Connecticut.
- Minimum 1 year of experience in home healthcare or a related field.
- Strong research and communication skills, with the ability to work independently and as part of a team.
- Knowledge of systems integration and electronic medical records.
