Job Title: Case Manager

We are seeking a highly skilled and compassionate Case Manager to join our team at Civility Home Care. As a Case Manager, you will play a vital role in coordinating services and providing resources to our clients, ensuring they receive the best possible care and support.

Key Responsibilities:
  • Assess client needs and develop strategies to address them
  • Provide support and resources to clients, including referrals to community services and government programs
  • Coordinate services with social workers, doctors, and other healthcare professionals
  • Evaluate the quality of services and ensure they meet client goals
  • Determine the best course of action to help clients, whether it be with a social worker, psychologist, or medical doctor
  • Create programs and service plans to ensure the best outcomes for clients
  • Listen to and resolve issues, providing advice and support to clients and their families
  • Care for the well-being of clients, including those with serious illnesses, addictions, disabilities, or who have been neglected
  • Organize and influence community and policymakers to develop programs and services that support social work
  • Advocate and raise awareness on behalf of clients and the community
Requirements and Qualifications:
  • Excellent written and spoken communication skills
  • Excellent interpersonal skills with colleagues, community leaders, policymakers, and others
  • Excellent time management skills, with the ability to prioritize tasks and manage multiple cases
  • Motivated to take on additional community involvement projects and solve problems
  • Comfortable working in a fast-paced environment with multiple cases
  • Able to organize and manage large amounts of files, schedules, and information
  • Self-directed and able to work without supervision
  • Empathetic and supportive, with mentorship and leadership skills
  • Proficient in Microsoft Office Suite (Word, PowerPoint, and Excel)
  • Able to work nights and weekends, and travel to meet with clients in MA, CT, and NY
