Job Title: Case Manager

We are seeking a highly skilled and compassionate Case Manager to join our team at Civility Home Care. As a Case Manager, you will play a vital role in ensuring the well-being of our clients by coordinating services, providing resources, and advocating on their behalf.

Key Responsibilities:
  • Assess client needs and develop strategies to address them
  • Provide support and resources to clients, including connecting them with community resources and government services
  • Research and refer clients to relevant resources, such as healthcare services
  • Assign workers to respond to shifts and ensure timely service delivery
  • Evaluate the quality of services and monitor progress towards goals
  • Determine the best course of action to support clients, whether it be through social work, psychology, or medical services
  • Create service plans to ensure optimal outcomes for clients
  • Listen to and resolve issues, providing guidance and support to clients, workers, and the agency
  • Care for the well-being of clients, including those with serious illnesses, addictions, disabilities, or who have been neglected
  • Organize and influence community and policymakers to develop programs and services that support social work
  • Advocate on behalf of clients and the community, raising awareness of their needs and concerns
Requirements and Qualifications:
  • Superb written and spoken communication skills
  • Excellent interpersonal skills, including the ability to work with colleagues, community leaders, and policymakers
  • Excellent time management skills, with the ability to prioritize tasks and manage multiple cases
  • Motivation to take on additional community involvement projects and solve problems
  • Comfortable working in a fast-paced environment with multiple cases
  • Able to organize and manage large amounts of files, schedules, and information
  • Self-directed and able to work independently
  • Comfortable building personal relationships and dedicated to helping others
  • Empathetic and supportive, with mentorship and leadership skills
  • Ability to maintain compliance with local, state, and regulatory bodies
  • Proficient computer skills, including Microsoft Office Suite
  • Able to work nights and weekends, and travel to meet with clients in MA, CT, and NY
