RN Case Manager

We are seeking a skilled RN Case Manager to join our team at Nuvance Health. As a RN Case Manager, you will play a critical role in providing high-quality patient care and ensuring the best possible outcomes for our patients.
Key Responsibilities- Assess the physical, emotional, and socio-economic needs of patients and their families, and develop individualized plans of care to address these needs.
- Provide direct skilled nursing care in accordance with physician orders and document care in a timely manner.
- Observe, evaluate, and report to the physician the patient's physical and mental condition, changes in condition, and termination of care when goals are attained or when termination of care is required according to agency policy.
- Provide patient, family, and caregiver counseling and education to promote optimal health outcomes.
- Participate in the agency's quality assurance and performance improvement plan, and agency-sponsored inservice training.
- Develop, implement, and evaluate plans of care in partnership with the patient, representative (if any), caregiver, and physician.
- Supervise and provide clinical direction to LPNs, and fulfill these responsibilities under the direction of the preceptor or clinical nurse manager within the first 90 days.
- Identify patient and family needs for other home health services and initiate referrals when appropriate under the direction of the clinical nurse manager.
- Fulfill all compliance responsibilities related to the position.
- Requires strong research skills, clear and concise interpersonal communication skills, and knowledge of systems integration.
- Minimum experience: one year.
- Desired: Bachelor's Degree in Nursing; experienced in qualified home health agency.
- Manual: significant manual skills/motor coordination and finger dexterity.
- Occupational: some occupational risk.
- Physical Effort: very heavy effort. May exert up to 50 lbs. force.
- Physical Environment: some exposure to dirt, odors, noise, human waste, etc.
