We are seeking a skilled RN Case Manager to provide community-based care coordination and management for our clients in Delaware. This is a full-time, hybrid position that requires residing within the state.
Key Responsibilities- Conduct face-to-face needs assessments with members in their homes, nursing facilities, and community-based settings.
- Develop and implement care plans to address members' chronic and complex healthcare needs.
- Coordinate care across the continuum of services, including physical, behavioral, and long-term services and supports.
- Facilitate transitions to alternate care settings, such as hospital to home or nursing facility to community setting.
- Provide education and resources to members and caregivers on healthcare needs, benefits, and services.
- Collaborate with healthcare and service delivery teams to coordinate care needs and community resources.
- 5 years of intensive case management and discharge planning experience.
- Experience working with geriatric special needs, behavioral health, and home health populations.
- Licensed Bachelor's Social Worker (LBSW) or equivalent.
- Ability to communicate clearly with members and team members over the phone and in person.
