The Role

We are seeking a highly motivated Care Coordinator to join our team. As a Care Coordinator, you will be the primary point of contact for clients seeking mental health care. Your role will involve providing exceptional support, navigating clients through our care system, and ensuring seamless access to therapy services.

Key Responsibilities
  • Provide primary support for clients seeking mental health care, addressing their needs and concerns in a compassionate and empathetic manner.
  • Utilize our technology platform to connect clients with the right therapist, ensuring timely and effective access to care.
  • Collaborate with external partners and healthcare systems to ensure seamless care coordination and referrals.
  • Conduct outreach to referred clients, providing personalized onboarding services and assistance with all aspects of getting into care.
