Palliative Care Nurse Practitioner Opportunity

We are seeking a skilled Palliative Care Nurse Practitioner to join our team at Alamo Hospice and Palliative Care. As a Palliative Care Nurse Practitioner, you will work collaboratively with our Palliative Care Medical Director and other team members to deliver high-quality palliative care to patients in their homes within a 60-mile radius of our San Antonio, TX office.

Responsibilities:
  • Provide regular visits to reassess patients' needs, concerns, and update their plan of care
  • Initiate communication with primary care physicians and other specialists/agencies involved in patient care
  • Prescribe medications, including controlled substances, to the extent delegated and licensed, in accordance with state law
  • Order and interpret laboratory and other diagnostic tests within the scope of professional practice
  • Order treatments and durable medical equipment as indicated
Requirements:
  • Graduate from an accredited registered nursing program
  • Licensed as a Nurse Practitioner in the state of practice
  • Minimum of two years of varied work experience as a professional nurse
  • Current CPR Certification
  • Demonstrated skill in nursing practices and clinical judgments
  • Ability to make home visits and provide nursing care in the home environment
  • Ability to cope with family emotional stress and tolerance of individual lifestyles
  • Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
  • Valid driver's license and proof of insurance
