We are seeking a board-certified Nurse Practitioner to join our Geriatrics and Supportive Care division at the Department of Family and Community Medicine, Joe R. and Teresa Lozano Long School of Medicine at UT Health San Antonio.
Key Responsibilities- Provide high-quality patient-centered care in nursing homes, assisted living, home visits, post-acute care, and outpatient clinic duties for geriatrics/palliative care.
- Collaborate with interdisciplinary teams to standardize and optimize primary care workflow and patient-centered primary care.
- Participate in the development and implementation of innovative team-based care to facilitate care in the hospital and in between the hospital, clinic, and other settings.
- Board certification as FNP or AGNP required.
- Clinical experience and interest in hospice and palliative services preferred.
- Effective communication skills; excellent health assessment skills; good computer skills; demonstrated ability to work effectively as a member of a collaborative health care team.
