Position Overview
We are seeking a qualified Palliative Care Nurse Practitioner to join our esteemed team. In this role, you will report directly to the Executive Director or Administrator and will be responsible for conducting Face-to-Face (F2F) assessments. Your primary duty will be to collect clinical data that supports hospice diagnosis and prognosis, which will be communicated to the attending hospice physician for certification purposes.
The responsibilities of the Palliative Care Nurse Practitioner include:
- Conducting thorough physical assessments for F2F encounters within the regulatory time frames set by CMS.
- Providing detailed clinical findings, including evaluations of medical treatment plans, diagnostic tests, lab results, and medication lists to the physician overseeing patient recertification.
- Making informed recommendations to the Interdisciplinary Group (IDG) based on F2F assessment outcomes.
- Informing the hospice agency promptly if unable to complete a scheduled F2F visit.
Qualifications
- Master's degree or higher in Nursing.
- Completion of an accredited graduate-level nurse practitioner program.
- Current state licensure as a registered nurse.
- Current state licensure or certification as an advanced practice nurse.
- National certification as a Nurse Practitioner (ANCC or AANP).
- Must possess a collaborative agreement.
