Work Schedule: Monday to Friday, 8 AM to 5 PM

Key Responsibilities:

  • Lead the assessment of patient eligibility for care in accordance with state and federal regulations.
  • Gather comprehensive data on the physical, psychological, social, and spiritual needs of patients and their families, facilitating interventions by the interdisciplinary team.
  • Coordinate communication with attending physicians and interdisciplinary staff to ensure optimal care and facilitate admissions or recertifications.
  • Utilize available resources effectively to meet patient needs.
  • Conduct regular visits to evaluate patient eligibility, assess needs, and update care plans accordingly.

Required Qualifications:

  • Graduation from an accredited nursing program.
  • Current Nurse Practitioner licensure in the relevant state.
  • A minimum of two years of diverse nursing experience.
  • Current CPR certification.
  • Proven skills in nursing practices and clinical decision-making.
  • Ability to perform home visits and deliver care in a home setting.
  • Capacity to manage family emotional stress and respect individual lifestyles.
  • Strong interpersonal, verbal, and written communication skills.
  • A valid driver's license and proof of insurance are mandatory.
