Job Summary

We are seeking a skilled and compassionate Nurse Care Coordinator to join our team at SSM Health. This role is a critical part of our mission to deliver exceptional patient care and support.

About the Role:

  • This position will work closely with patients, families, and healthcare teams to ensure seamless transitions of care throughout the continuum.
  • You will assess patient needs, develop personalized plans, and coordinate services to promote optimal health outcomes.
  • A strong understanding of the nursing process and ability to navigate complex healthcare systems are essential for success in this role.
Responsibilities
  • Assess patient needs and develop comprehensive care plans in collaboration with interdisciplinary teams.
  • Carefully coordinate referrals, appointments, and follow-up care to ensure timely and effective treatment.
  • Communicate effectively with patients, families, and healthcare providers to ensure clear understanding of care plans and progress.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed to achieve optimal outcomes.
  • Maintain accurate and up-to-date records of patient interactions, care plans, and outcomes.
Requirements
  • Degree from an accredited school of nursing or equivalent experience.
  • Minimum 3 years of registered nurse experience, preferably in a clinical setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • BLS certification and RN licensure in applicable states.
