Job DescriptionWe are seeking a skilled Registered Nurse to join our Float Pool team as a Nurse Care Coordinator. In this role, you will provide high-quality patient-centered care, collaborating with an interdisciplinary team to assess and address patient needs.
You will be responsible for:
- Conducting comprehensive assessments of patients' conditions and developing individualized plans of care;
- Collaborating with the care team to prioritize patient needs and coordinate care delivery;
- Providing education and support to patients and families on disease management, medication use, and health promotion;
- Maintaining accurate and timely documentation of patient care activities;
- Promoting a culture of safety and quality within the organization.
To be successful in this role, you must have:
- A current RN license in the state of Connecticut;
- At least one year of experience in a hospital or healthcare setting;
- Strong communication and interpersonal skills;
- The ability to work effectively in a fast-paced environment and prioritize tasks efficiently;
- Basic computer skills and proficiency in electronic medical records systems.
