Key Responsibilities- Conduct comprehensive psychosocial assessments to identify patients' needs and develop individualized plans.
- Collaborate with the interdisciplinary team to ensure seamless discharge planning and coordination of post-hospital services.
- Provide crisis intervention and short-term supportive counseling to individuals and families experiencing temporary or situational problems.
- Act as a liaison between patients, families, and community agencies to ensure access to necessary resources and services.
- Document all interactions, assessments, and interventions in medical records.
- Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) through the Louisiana State Board of Social Work Examiners.
- Experience working in a healthcare setting, preferably in a hospital or acute care environment.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Flexibility to work variable hours, including evenings and weekends, as needed.
