Job Summary

We are seeking a highly skilled Licensed Masters Social Worker to join our team at Ochsner Health. As a key member of our multidisciplinary team, you will be responsible for conducting bio-psychosocial assessments, providing patient education, and coordinating discharge plans.

Key Responsibilities
  • Conduct thorough patient assessments to identify needs and develop effective care plans
  • Provide education and support to patients and families to promote health and well-being
  • Collaborate with the healthcare team to coordinate discharge plans and ensure a smooth transition for patients
  • Maintain accurate and detailed patient records, documenting all interactions and interventions
  • Stay up-to-date with the latest clinical knowledge and best practices in social work
Requirements
  • Master's degree in Social Work (MSW) from a Council on Social Work Education (CSWE) accredited university
  • Current licensure as a Licensed Masters Social Worker (LMSW) or ability to obtain within 6 months of employment
  • 2+ years of experience in a medical setting, preferably in case management or social work
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
