Responsibilities
  • Provide and coordinate care to patients and families in the clinical command center
  • Collaborate with healthcare teams to ensure effective care transitions and patient outcomes
  • Use critical thinking and problem-solving skills to address patient needs and concerns
  • Communicate effectively with patients, families, and healthcare teams through various technologies and platforms
  • Participate in quality improvement initiatives to enhance patient care and safety
  • Stay up-to-date with department policies, workflows, and continuing education requirements
Requirements
  • Current RN licensure and a minimum of 5+ years of recent nursing experience
  • Knowledge of care transitions and electronic medical records (EMR)
  • Ability to work effectively in a matrixed environment and build alliances with various stakeholders
  • Strong communication and problem-solving skills, with the ability to work in a fast-paced environment
  • Valid Basic Life Support (BLS) certification and knowledge of Advanced Life Support (ALS) (preferred)
