About the Role

AID Upstate Inc. is seeking a highly skilled and compassionate Medical Case Manager to join our team. As a Medical Case Manager, you will play a vital role in connecting individuals affected by HIV to vital services and helping them navigate both personal and healthcare-related challenges.

Key Responsibilities
  • Assist clients in accessing and maintaining benefits programs to ensure they receive essential care and treatment.
  • Prepare client histories, action plans, and service reports to ensure comprehensive and client-centered care coordination.
  • Work closely with clients through individual or group conferences to help them understand and follow medical recommendations.
  • Collaborate with physicians, nurses, and other healthcare providers to create optimal treatment plans.
  • Stay up-to-date on available community resources and services to effectively connect clients to what they need.
  • Document all client-related activities following AID Upstate and DPH policies.
  • Manage a caseload of 100+ individuals, ensuring all client information remains confidential, even after employment.

Requirements
  • A Bachelor's degree in behavioral science or a related field, with at least two years of relevant experience (or equivalent education/experience).
  • Fluency, both written and spoken, in English and Spanish is required.
  • Proficiency in Microsoft Office (Word, Outlook) and familiarity with office equipment like phones, copiers, and fax machines.
  • Strong interpersonal skills and the ability to work with diverse populations.
  • Ability to maintain confidentiality and comply with HIPAA regulations both during and after employment.
