Job Summary: We are seeking a skilled Hospice RN Case Manager to join our team at Lutheran Homes of South Carolina. As a Hospice RN Case Manager, you will play a vital role in providing high-quality, patient-centered care to individuals and families in their homes and communities.

Key Responsibilities:

  • Plan, organize, and direct hospice care services to meet the unique needs of each patient and family.
  • Collaborate with the Interdisciplinary Team to develop and implement individualized care plans.
  • Provide education and support to patients and families on end-of-life care and related topics.
  • Conduct regular assessments and evaluations to ensure the highest level of care and quality of life for patients.
  • Supervise and mentor hospice aides to ensure they are providing high-quality care.

Requirements:

  • Graduate of an accredited school of nursing with a current RN license in the state of South Carolina.
  • Minimum of one to two years of nursing experience in an acute care setting, with recent experience in home care preferred.
  • Excellent observation, verbal, and written communication skills, as well as problem-solving skills and basic math skills.
  • Ability to lift, position, and/or transfer patients, as well as lift supplies and equipment.
  • Reliable transportation for daily travel throughout the service area.
