The Inpatient Unit Registered Nurse Case Manager is a vital member of our interdisciplinary team, responsible for delivering high-quality patient care and promoting the Compassus philosophy. This role requires a strong nursing background, excellent communication skills, and a passion for providing compassionate care to patients and their families.
Key Responsibilities- Coordinate clinical, psycho-social, and spiritual services for patients through case management
- Collaborate with the interdisciplinary team to meet patient needs and develop effective care plans
- Provide education and support to patients and families, promoting their maximum level of comfort and coping
- Ensure accurate and timely documentation of patient information and care plans
- Perform supervisory duties for Hospice Aides and Licensed Practical/Vocational Nurses, promoting ideal patient outcomes
- Communicate effectively with the interdisciplinary team, Medical Director, and Attending Physician to ensure seamless care coordination
- Active and unencumbered Registered Nurse license in state(s) of employment
- Current CPR certification
- Minimum of one (1) year nursing practice or equivalent experience
- Hospice, oncology, home health, and/or long-term care experience preferred
The Inpatient Unit Registered Nurse Case Manager will work in a fast-paced healthcare environment, requiring strong critical thinking and problem-solving skills. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds.
Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
