Key Responsibilities:

- Collaborate with healthcare providers, family members, and caregivers to ensure effective communication and coordination of care.
- Monitor patients' progress and adjust care plans as needed to ensure optimal health outcomes.
- Provide education and support to patients and their families to promote healthy behaviors and self-management of chronic conditions.
- Facilitate transitions of care between healthcare settings, including hospitals, skilled nursing facilities, and home health agencies.
- Document patient information and care plans in a timely and accurate manner.

Requirements:
- Current, unrestricted Registered Nurse license in the state of assignment.
- Certification as a Registered Nurse by a national credentialing body.

- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
