Nurse Care Coordinator

The Licensed Practical Nurse in our Assisted Living community is responsible for delivering high-quality resident care by utilizing various nursing competencies such as assessments, care planning, and implementation of interventions. This individual will also support Certified Nursing Assistants (CNAs) to ensure resident care standards are met and maintained. Confidentiality and adherence to HIPAA regulations are essential.
Shift Details: PRN (Assisted Living operates 8-hour shifts).
The ideal candidate will possess strong clinical skills, be able to care for geriatric residents, and have the ability to assess, implement, evaluate, and re-evaluate resident care needs. Key responsibilities include providing direction and assigning daily duties to nursing staff, serving as a resource for staff concerning resident care and needs, and keeping management informed of the status. Additionally, this nurse will implement physician orders, administer medications and treatments, participate in Continuous Quality Improvement (CQI) activities, document pertinent information in resident charts, meet all documentation standards, and utilize new technologies to impact productivity.
This role requires collaboration with residents, families, and team members to formulate individualized care plans, working collaboratively with the team and management to coordinate care and manage outcomes, and implementing appropriate care to meet spiritual, physical, psychological, and social needs. As an advocate for residents, staff, and others, this nurse must treat others with respect and possess a positive attitude, initiative, and the ability to multi-task and work as part of a team or independently.
Required Qualifications:
- A valid license as a Practical Nurse in the State of Virginia in good standing;
- CPR, AED, and First Aid Certification;
We estimate the salary for this position to be between $55,000 to $70,000 per year, depending on experience.
