Job Summary

We are seeking a highly skilled Care Navigator to join our team at Homeward. As a Care Navigator, you will play a critical role in helping our members achieve their best health by providing proactive outreach, building relationships, and connecting them with necessary services.

Key Responsibilities
  • Member Engagement: Conduct telephonic, video, and in-person outreach to build relationships with members and connect them with medical, behavioral, and social services.
  • Care Planning: Work with members and their providers to develop and implement care plans that address their unique needs and goals.
  • Resource Navigation: Identify and maintain a list of community resources and contacts to meet our members' needs.
  • Documentation: Document all member encounters in our designated electronic platform in a timely manner.
  • Collaboration: Collaborate with members' primary care providers and their teams to ensure cohesive care.
Requirements
  • Education: Completion of a Medical Assistant program from an accredited program or school.
  • Experience: At least two years of experience in high-touch, patient-facing roles, preferably with seniors.
  • Skills: Passion for delivering care in rural America, ability to persuade members to take actions that support their health, and expertise with the local community, geography, culture, healthcare ecosystem, and available resources.
