CentraCare is seeking a dedicated full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our collaborative healthcare team in a Primary Care setting.
Key Responsibilities
As an Advanced Practice Provider, you will establish and manage your own patient panel, providing comprehensive care to individuals across all age groups. Your duties will include:
- Diagnosing and treating acute and chronic health conditions.
- Delivering preventive services such as routine examinations, health-risk evaluations, immunizations, and screening procedures.
- Offering tailored guidance on healthy lifestyle choices.
- Coordinating care for patients with ongoing health issues.
- Supporting the walk-in clinic and fulfilling additional responsibilities as needed, including nursing home visits and hospital duties.
Work Schedule
- Full-time commitment of .8-1.0 FTE
- Flexible options for a four or five-day workweek
- Participation in Saturday morning clinics and hospital rounds
- No evening shifts or on-call duties
To qualify for this role, candidates must possess:
- Graduation from an accredited nurse practitioner or physician assistant program.
- Certification as a Nurse Practitioner or Physician Assistant.
- Current or anticipated licensure in the relevant state, with the ability to treat patients from pediatrics to geriatrics.
- Basic Life Support (BLS) Certification; Advanced Cardiovascular Life Support (ACLS) and Comprehensive Advanced Life Support (CALS) certifications are preferred.
