Job Summary

We are seeking a highly skilled Licensed Practical Nurse, LPN to join our team at Lowell General Hospital. As a key member of our healthcare team, you will provide professional and non-professional nursing care services to patients in accordance with physician orders.

Key Responsibilities
  • Implement medical/nursing plans of care, nursing interventions, medication administration, and procedures necessary for patient care under the direction of a Registered Nurse.
  • Provide direct patient care within the primary nursing model to ensure continuity of care for all patients within your caseload in a safe and efficient manner.
  • Perform technical aspects of patient care (medications, treatments, etc.) safely, according to established policies and procedures and Nurse Practice Act.
  • Assess and reassess pain, utilizing appropriate pain management techniques, and document pain management.
  • Communicate all changes in patient condition immediately to the Registered Nurse.
  • Bases all nursing interventions on pathophysiological and psychosocial rationale, delivering care in a fiscally responsible manner.
  • Establish a positive relationship with patients and families, introducing yourself to patients and families as the patient's nurse, providing teaching and counseling for patients/families according to their needs and ability to understand, and documenting teaching on the appropriate teaching tool.
  • Review/educate discharged patients and families on ADLs, medications, treatments, and provide appropriate discharge instruction sheet(s).
  • Document all nursing interventions and patient response in the medical record for all assigned patients.
  • Delegate responsibilities to others with an understanding of their roles, knowledge, and capabilities.
  • Participate in performance improvement activities as requested, utilizing performance improvement principles to support and improve patient care.
  • Hold yourself accountable for professional practice and present yourself in a calm and professional manner, keeping current with changing practices and interventions in patient care through lifelong learning.
  • Maintain a collaborative work relationship with peers and colleagues in order to create a positive work environment.
  • Report pertinent observations and reactions regarding patients to the appropriate person (i.e., physicians, clinical manager, or charge nurse) and record those observations accurately and concisely.
