Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will have the opportunity to work with a variety of patients and provide high-quality care in a fast-paced environment.

Key Responsibilities
  • Deliver post-acute care to residents, including monitoring their physical and mental wellbeing and tracking their vital signs.
  • Perform intakes of residents and document their health concerns and medical histories.
  • Review medical records and charts of residents to ensure accurate and up-to-date information.
  • Help maintain appropriate levels of medical supplies and contribute to a safe and comfortable environment for residents.
  • Assist with IV set up, vaccinations, and phlebotomies as needed.
  • Be accountable for following and adhering to care plans for residents.
  • Help medical staff with orientation and training as needed.
