Transforming Care Transitions

As a Care Coordinator Associate at MultiCare Health System, you will play a vital role in ensuring seamless transitions for patients with complex healthcare needs. This position requires a collaborative approach, excellent customer service skills, and the ability to utilize multiple data platforms to facilitate smooth care transitions.

Key Responsibilities

  • Assist in the cross-continuum approach to hospital transitions, working closely with physicians and clinical professionals to achieve collaborative practices.
  • Utilize technology to troubleshoot and resolve issues related to scheduling, insurance, and authorizations.
  • Develop and maintain strong relationships with patients, families, and healthcare teams to ensure effective care coordination.
  • Stay up-to-date with changing healthcare regulations and best practices to provide high-quality care.

Requirements

  • Post-secondary coursework in a health-related field, with a bachelor's degree highly desirable.
  • Minimum of 2 years of experience in a healthcare setting, preferably as a Health Unit Coordinator (HUC) or in a clinical role such as medical assistant, certified nursing assistant, or EMT.
  • Experience in utilization management, community resources, and/or financial and quality issues affecting patient care is highly desirable.

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental, and retirement benefits, and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $32.72 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant's years of experience align.
