About the Role

We are seeking a highly skilled Care Coordinator Associate to join our interdisciplinary care team. As a key component of our hospital transitions approach, this role will play a critical part in achieving collaborative practices that exemplify MultiCare Health System's commitment to patient-centered care, community engagement, and focus on transition of care for patients with physical and behavioral healthcare needs.

Key Responsibilities
  • Collaborate with physicians and clinical professionals to ensure smooth throughput and care transitions for patients.
  • Utilize multiple data platforms to troubleshoot and resolve issues as needed.
  • Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care transitions.
  • Stay up-to-date on relevant healthcare policies, procedures, and best practices to provide high-quality care.
Requirements
  • Post-secondary coursework in a health-related field.
  • Minimum of 2 years of experience in a healthcare setting, preferably as a Health Unit Coordinator (HUC) or in a clinical role such as medical assistant, certified nursing assistant, or EMT.
  • Experience in utilization management, community resources, and/or financial and quality issues affecting patient care is desirable.
